Statutory Declaration Format

Hello everybody!
I'm getting married abroad, but I need to write a statutory declaration of civil status in Canada before leaving before a Commissioner of Oaths .. I should mention my current marital status and citizenship for my spouse. Does anyone know how to write a statutory declaration and in what format should I use? Thank you!
