Name Change Procedure In India

My son was born in the birth certificate belongs to orissa.His orissa.His passport will be put in Andhra pradesh.I have some questions -
1) Can his father changed his name in Andhra then apply for a passport?
2) Can we change the name in the birth certificate issued by a State in another State?
3) How can I change the name in the passport for minors? What is the procedure for the same?
