Lost copy of legal name change - Arizona how to get another copy? The legal name change (last) was obtained by the grandmother of my son. I contacted her to obtain a copy of the document to update school records, etc. ssn but is unable to locate the document and does not remember the name of the lawyer or filed with the court. I do not know where to get started copying.
What county your grandmother alive at the name change? This would be the starting point. What was the name of your son at the time? You should be able to find documents on this basis.

Try the courthouse in the city, she lived at the time.
Go to your local Social Security Administration to obtain a copy.
